Uploading Referral Documents
Supporting documents — most commonly a signed consent form — can be attached to a Referral as part of the intake process.
Where to upload
Document upload sits in a collapsible section beneath the consent form on Step 2 (Consent) of the Referral wizard.
Documents attach to a Referral record, and that record is only created when you click Continue at the end of Step 1. If you have not got past Step 1 yet, there is nothing to attach the file to.
How to upload
- Open the documents section on the Consent step
- Drag a file onto the drop zone, or click it to browse
- Wait for the upload to confirm before leaving the page
Accepted file types

Maximum file size: 10 MB per file. Files that are too large, or of a type not listed above, are rejected with a message explaining why.
A photographed or scanned consent form is fine — images are accepted. If a scan comes out larger than 10 MB, reduce the resolution or save it as a PDF rather than splitting it into multiple files.
Signed consent documents
The signed consent form is tracked separately from the consent checkboxes on the form. Ticking the consent boxes records that consent was obtained; uploading the document provides the evidence.
If no signed consent document has been uploaded by the time you dispatch, the dispatch dialog shows a reminder. This does not block dispatch — but the document stays outstanding against the Referral until it is provided.
